DeQuervains tenosynovitis is a pain resulted from irritation or inflammation of the wrist tendons at the base of the thumb. Pain is experienced with movement of the thumb. This inflammation is called ‘stenosing tenosynovitis” This simply means that the inflammation involves the synovium and tendons that control the movement of the thumb.

How is DeQuervains Stenosing Tenosynovitis Diagnosed?

Typically tenderness directly over the involved tendons on the thumb-side of the wrist is the most common finding. A test is generally performed in which the patient makes a fist with the fingers clasped over the thumb. The examination of deQuervain’s is usually quite typical with tenderness over the involved tendons.

For this condition, one specific maneuver called ‘Finkelstein’s test’ is quite specific. Finkelstein’s test is performed by making a fist and moving the wrist towards the small finger. Patients with deQuervain’s tenosynovitis will report pain with this maneuver.

What are the Causes?

DeQuervain’s tenosynovitis is caused by an irritation of the tendons at the base of the thumb, usually caused by taking up a new, repetitive activity.

Non-operative treatments include:

  • Resting the thumb and wrist by wearing a splint,
  • Oral anti-inflammatory medication,
  • Cortisone-type of steroid injected into the tendon compartment,

 

The goal is to relieve the pain caused by the irritation and swelling. In some cases, simply stopping the aggravating activities may allow the symptoms to go away on their own.

Surgery may be recommended if symptoms are severe or do not improve. The surgery opens the compartment to make more room for the inflamed tendons, which breaks the vicious cycle where the tight space causes more inflammation.
